V.A. Computer Glitch

Computer software glitches since August have exposed patients at Veterans Affairs hospitals to incorrect doses of drugs and other medical errors. V.A. documents obtained through the Freedom of Information Act find no evidence any patient was harmed. The V.A. says it's still reviewing the situation.
